Enter your username, password, and CAPTCHA code to log in if you are already registered. If you are not registered, click the “Don’t have account, Register as a new user” option.

After you log in, click on the “Forms” tab and then “Form 6B”.

Enter your EPIC number, Aadhaar number, and mobile number.

Click on the “Preview” so that you can review your application.

If you are satisfied with your application, click the “Submit” button.

You will receive a reference number for your application.
